Tiger Center for Health
The Tiger Center for Health is a school-based health center that serves the students and faculties of Chapmanville Regional High, West Chapmanville Elementary, East Chapmanville Elementary, Chapmanville Middle, and Hugh Dingess Elementary Schools. In the near future, all students and faculty in Logan County will be able to utilize the Tiger Center. There are over 40 school-based health centers throughout WV. Nationally, almost every state in the nation has school-based health centers. The mission of these centers is to improve the health of students and faculty, increase access to health care, and decrease time lost from school by providing health care within the school setting. the Logan County Board of Education and Southern WV Health System have joined together to provide both medical and dental services within the school setting.
The Tiger Center offers a variety of services. As the program grows, we hope these services will continue to meet the needs of students and families. Services currently available are:
1. Assistance in dealing with medical emergencies at school
2. Medical Visits for:
--Over-the-counter medicines and first-aid to help students/faculty remain in school
--Allergy shots with a doctor's order
--Screening Exams (vision, hearing, blood pressure, etc.)
--Acute illness or injury
--Follow-up for long-term illness
--health Check Exams, Physical Exams, Sports Physicals
3. Dental Visits for:
--Exam, Cleaning, Sealants, Fluoride Treatment, X-Rays
4. Health Education
--special programs and presentations in the school and community
--Individual health education during visits
--Library of patient handouts
Parents of the parents of the participating students must complete the consent form and health history. This information gives us permission to treat your child, provides us with important medical and dental information, and gives us contact information to reach you when necessary, your consent form will follow your child through school, but we will ask you to complete a brief update form each year. Please be sure to contact the Tiger Center if anything about your child's medical history, insurance information, or emergency information changes. If you decide you no longer want your child to use the Tiger Center, be sure to contact us in writing to withdraw your consent. your child may give his/her own consent at age 18. your child may request to come to the tiger Center or your child's school may refer him/her to us. your child will be evaluated and then either treated or scheduled with a provider as appropriate. Physicals and sports physicals should be scheduled ahead of time whenever possible.
The Tiger Center is sustained by revenue from the services that we provide. If your child has insurance coverage, SWVHS and Tiger Center will submit charges to your insurance for billing purposes. We accept Medicaid, Chips, and most private insurances. No one will be denied services based on the inability to pay.
Your confidentiality and privacy are important to us. Federal law requires us to inform you of our agency's Notice of Privacy Practices.
